项目摘要

平成18年度、19年度に引き続き、認知症高齢者の専用フロアをもつ精神科病棟にて、認知症高齢者の意思決定代理人のインフォームドコンセントによる許可を得たのち、施設利用者および介護職員(フロア担当者等、昼夜交代あり)にICタグを装着して、行動観察を行った。居住スペース内のベッド、トイレ、廊下、いこいの場などにトリガー領域を設置し、長期間、昼夜24時間連続モニタリングを実施し、行動観察をおこなってきた。利用者の毎日のベッド内滞在時間、トイレ回数、歩行距離などの行動パターンと病態との関係を把握することが可能となり、徘徊行動のおこるパターン解析、徘徊行動をおさえるための介入研究をおこなった。また、1日の行動記録の自動化を試み、自動日報作成のための生活行動分類をおこなった。また、介護者の行動の記録をとるために、ICレコーダで介護行為を記録することとした。しかしながら、現在の音声認識技術では、会話文の自動テキスト化は困難であったので、介護者と利用者の会話を定型化することを考えて、介護現場で必要な「声掛け運動」を提唱し、いろいろな場面での会話を体系化した。このことは、単に、自動記録ができるだけでなく、介護者と利用者のコミュニケーション(必要な場面で、必要な言葉をかける)のに役立ち、さらに、介護者が質の高い介護を実践するための研修にも役立つことがわかった。
