项目摘要

高齢者介護施設入居者の衣生活の質を向上させ、要介護高齢者の精神状態や日常生活動作を維持または改善させる衣生活支援ができる人材(ファッション・セラピスト)養成の必要性を実証することを目的として、職員が日常的に着装アドバイスを行っている施設と行っていない施設で、6ヶ月間ファッション・セラピーを実施し、終了1年後におけるセラピーの効果の持続性を検討した。日常的に着装アドバイスを行っている施設のA氏(開始時90歳、女性、要介護度2)、B氏(86歳、女性、要介護度3)、C氏(73歳、女性、要介護度2)は、セラピー実施による効果が観察法では認められたが、職員による老人精神機能評価尺度(NMスケール)と生活能力評価には変化が見られなかった。セラピー終了1年後の職員による評価をみると、B氏は1年前の状態を維持しており、A氏は生活能力を維持、NMスケール合計点は-2点、C氏はNMスケール合計点は-1点、生活能力評価合計点は-3点であった。日常的に着装アドバイスを行っていない施設のD氏(89歳、女性、要介護度3)とG氏(74歳、男性、要介護度2)は、セラピー実施によりNMスケール・生活能力評価ともに改善が認められたが、セラピー終了1年後のNMスケール合計点はD氏-24点、G氏-16点、生活能力評価合計点はD氏-7点、G氏-7点低下した。E氏(83歳、女性、要介護度4)は、セラピー実施により生活能力評価のみ改善し、F氏(76歳、女性、要介護度3)は、NMスケールのみ改善したが、セラピー終了1年後のNMスケール合計点はE氏-16点、F氏-8点、生活能力評価合計点はE氏-10点、F氏-1点低下した。これらの結果より職員が日常的に衣生活支援を行うことにより、要介護高齢者の精神状態や日常生活動作の機能低下を緩やかにすることができると考えられ、ファッション・セラピスト養成の必要性が実証できた。
