项目摘要

小離島(島人口500人未満)は、交通・物流が未発達であり本土とは違うライフスタイルが予想されるが、未だ詳細な調査報告はほとんど無い。そこで我々は食習慣と動脈硬化の関連に注目し調査を行った。長崎県五島市の小離島である嵯峨島(人口238人)、椛島(人口278人)、黄島(人口58人)より149名の血中ホモシステイン(Hcy)測定とその代謝に関連するメチレンテトラヒドロ還元酵素(MTHFR)遺伝子多型を解析した。動脈硬化の指標として頸動脈内中膜複合体厚(IMT)と心臓足首血管指数(CAVI)を測定した。また五島市の福江島(人口43,331人)と本土の雲仙市(人口52,230人)のデータと比較した。小離島の食習慣の調査としては、椛島で11月と2月に2回、延べ52人に、栄養士・医師が各家庭を訪問もしくは調査票に自己秤量・記入方式にて行った。Hcyに影響を及ぼす因子で調整したHcyは、小離島12.1±0.3μmol/l(平均±標準誤差)、福江島8.5±0.1μmol/l、雲仙市7.4±0.3μmol/lと有意に(p<0.001)差を認めた。MTHFR遺伝子多型の頻度、IMT、CAVIは3つの地域で差は無かった。小離島では、平成16年国民栄養調査と比し、魚介類が2倍近く摂取され、食塩の摂取も多かった。Hcyに影響を与える栄養素では、葉酸408μg、ビタミンB_61.6mg、ビタミンB_127.1μgと十分量摂取されていた。事前の簡易調査から魚介類を除いた生鮮食品の摂取が少ないと予想したが、各家庭に大型冷凍庫を備え、家庭菜園を活用し十分な食材を摂取していた。今回の調査は例数が少なく、1〜3日間の短期調査であるため、十分に小離島の食習慣を表していない可能性があるが、初めての小離島における栄養学的データが得られた。Hcyがへき地ほど高くなる原因は、今後も調査が必要であると考えられた。
