项目摘要

本研究は、診療時間外となる夜間・休日の在宅糖尿病患者の看護ニーズを明らかにし、糖尿病療養指導士を中心とした電話等による相談対応システムの構築の検討を目的とする。平成19年度に実施した1)糖尿病専門医が診療する全国の病院の看護管理者に対する質問紙調査、2)研究協力者が所属するA糖尿病専門病院の病棟看護師(日本糖尿病療養指導士を含む)へのグループ・インタービューを踏まえ、平成20年度は、夜間・休日の電話等による相談に病棟看護師が初回対応しているA病院において、夜間・休日に電話をしてきた患者のうち同意を得られた患者に対し、対応指導後の同日に、対応した看護師が患者宅に電話し、患者状況を確認のうえ必要に応じて再指導するという看護介入を試行した。本試行は、患者のセルフケア・フォローアップのシステム化を図る基礎調査の目的で、冬期の1月から2月に行い、看護部の承認を得て、日勤業務終了後の超過勤務とし、有謝礼での参加看護師を募集した。介入の評価は、研究者の作成項目に看護師判断で記入する方法とした。調査は、当初の予想に反して、超過勤務時間内に想定していた件数の電話相談がなく、期間を延長して夜勤勤務内でのボランティア参加協力を得たが、断られるケースや病棟業務の都合上介入が不可なケースも多く、分析可能なデータが集積していないため、現在も50件前後を目途に介入試行データを収集中である。まとめとして、在宅糖尿病患者の夜間・休日の電話等による相談はインスリン投与や低血糖などの療養上の相談が少なくないこと、対応には糖尿病の専門的知識が必要であり、糖尿病療養指導士のような教育指導的な要素が求められることから、電話相談の対応を夜間・休日の緊急一時的対応とするよりも恒常的なセルフケア・フォローアップとして外来看護に包括する業務システム化を図る方向性や病院-地域間の看護フォローアップを模索する必要が考察される。

The purpose of this study was to discuss the structure of the care system for in-home diabetic patients outside the clinic hours, at night and on weekends, at the clinic, at the center of diabetes care, at the telephone, etc. 1) A questionnaire survey conducted by a diabetes department doctor for diagnosis and treatment of diabetes mellitus among nursing managers in hospitals nationwide, 2) A diabetes department nurse affiliated with a research collaborator (Japanese Diabetes Care Counselor) The patient's condition is confirmed and re-guided.
