昨年度は、臨床経験の有無、上腕に力学的負荷をかける等の条件設定を行い、小サンプルを用いて歯学領域における臨床手技の運動をマクロ的に数理解析して、運動制御学的パラメータが手技の巧緻性を定量評価する上で有効であることを明らかにした。引き続き今年度は、サンプル数を増加させ、臨床手技の運動の詳細を表現する運動制御学的パラメータを追加して実験を行った。3年以上の矯正歯科の臨床経験を有するもの(熟練者群)と、矯正歯科臨床経験が3年未満のもの、あるいは全く矯正歯科臨床経験を有さないもの(非熟練者群)を被検者として、ファントムを用いて、利き腕にてピンセットを用いて人工歯表面の最適な位置にエッジワイズ装置(ブラケット)を接着する操作を行わせ、1)運動効果器(ピンセットの先端)の運動軌跡、2)頭部、上半身、上腕および基節、中節、末節骨の運動軌跡を同時記録した。運動制御学的パラメータとしては姿勢制御と、ハンドインスツルメント(運動効果器)の運動時間、運動速度、前後、横、垂直方向の変位量、円滑性および速度プロファイルの対称性を用いた。統計学的比較にはMann-Whiteney-U-testを用いた。熟練者群は、非熟練者群と比較して、減速相において、ハンドインスツルメントの運動の円滑性は有意に高く(P<0.01)、より対称性のある速度プロファイルを有することが示された(P<0.01)。さらに、熟練者群は、非熟練者群と比べて、加速相での前後方向の変位量が大きく、減速相では小さいという結果が得られた(P<0.01)。以上により、減速相での運動の円滑性および前後方向の変位量、速度プロファイルが手技の熟練度を評価する上で有効な指標であることが明らかとなった。これらの手技の熟練度が評価可能な運動制御的パラメータを明らかにすることで、技術の習熟度を客観的に評価してフィードバックする教育システム開発の基盤が構築されたと考えられる。

The above is a list of indicators for evaluating hand proficiency in the movement of deceleration phases and forward and backward directions.
