项目摘要

本研究の目的は、患者に渡す文書の正確な自動点訳が可能なシステムの構築と、病院内での利用法の確立である。今年度は、1.自動点訳プログラムeBrailleの辞書を拡張して、新たに医療文書の点訳に特化した“医Braille"の開発、2.本学医学部附属病院の外来案内・入院案内を点訳し点字パンフレットの作成、3.附属病院での運用に向けた体制作りと運用、を行った。まず、辞書への医療用語の追加では、例えば「上部心臓型総肺静脈還流異常症(ジョウブシンゾウガタソウハイジョウミャクカンリュウイジョウショウ)」等の長い文字列の語(複合名詞)を、より小さい単位の構成要素(名詞)に分割した上で、語の出現頻度に応じたコスト値を付与した辞書を作成した。医療用語の辞書への追加により“医Braille"を構築し、医療文書の点訳精度も向上した。次に、外来案内・入院案内の点字パンフレットを作成した。まず、点訳前に原文(漢字仮名混じり文)を、記載内容の重複の削除、尊敬語・丁寧語・謙譲語を避け平易な文へ、箇条書き、等により書き直した。そして“医Braille"で点訳し、点字パンフレットを試作した。その際、視覚障害者のコメントを基に改良した。そして、病院内での運用体制を整え患者からの問い合わせへも対応可能にすると共に、完成した点字パンフレットは誰でも自由に閲覧可能なように病院内に設置した。以上の取り組みは、視覚障害者や病院関係者から肯定的なコメントを頂戴すると共に、新聞やテレビ等のマスコミでも好意的に報道されている(神戸新聞、NHKニュースなど)。今後は、高度先端医療、ゲノム医療に於いて必然的に帰結する「個別化医療」への対応に向けて、点字による患者向けの個別の医療情報提供の他、病状説明の文書に記載する臓器などの図も点図で提供することを検討したい。また、各診療科との連携体制も整備していきたい。
