本年度は主として,種々の慢性肝疾患において肝局所に浸潤するリンパ球のT細胞抗原レセプターV領域usageを,7種類の抗V領域モノクローナル抗体を使用し免疫組織学的に検討した。用いた抗体は,抗Vα_2,抗vβ5.1,抗Vβ5.2+5.3,抗Vβ5.3,抗Vβ6.7,抗Vβ8subfamily,抗Vβ12.1およびLeu4(抗CD3),Leu2a(抗CD8),UCHL1である。C型慢性肝炎(HC)15例,B型慢性肝炎(HB)12例,原発型胆汁性肝硬変(PBC)5例,自己免疫性肝炎(AIH)3例の計35例の凍結組織切片を用い,酵素抗体間接法にて染色した。結果:肝内浸潤リンパ球のうち,Vβ5.1陽性細胞の比率はHCで18.9±12.3%(平均±標準偏差)であり,HB(5.7±6.0%)やAIH(1.3±1.2%)より有意に(P<0.05)高率であった。PBC症例では,5例中1例においてのみVβ5.1,陽性細胞が66%と高率に認められたが,他の4例においてはその陽性率はいずれも10%以下であった。Vβ5.1陽性細胞は主として門脈域に局在しており,その分布はUCHL1あるいはLeu2a陽性細胞と類似していた。一方,他のV領域に対する抗体の陽性率の平均は,いずれの疾患においても5%以下であった。次に,末梢血リンパ球におけるこれら抗体の陽性率をフローサイトメトリーにて解析(HC10例,HB5例,PBC7例,AIH5例,健常人5例)したが,各種V領域陽性細胞の平均比率は,いずれの疾患においても5%以下であり,健常人リンパ球との間に差は認められなかった。以上より,HCとPBCの一部の症例ではVβ5.1陽性細胞が選択的に肝内に集積しているものと考えられる。また,Vβ5.1陽性細胞とUCHL1あるいは,Leu2a陽性細胞の分布が類似していたことより,同細胞は主としてmemory cytolytic T lymphocytesであることが推定され,これらの疾患において病因学的に重要な意義を有していると考えられた。現在,PCR法を用いて更にこれらの点を検討中であり,平成5年の日本肝臓学会総会で発表する予定である。

This year, the use of 7 types of anti-V-domain T cell antigens and immunohistologic studies were carried out in patients with chronic liver diseases. Antibodies against Vα_2, Vβ5.1, Vβ5.2+5.3, Vβ5.3, Vβ6.7, Vβ8subfamily, Vβ12.1, Leu4(anti-CD3),Leu2a(anti-CD8),UCHL1 were used. Among them, 15 cases of chronic hepatitis C (HC),12 cases of chronic hepatitis B (HB),5 cases of primary biliary cirrhosis (PBC),3 cases of autoimmune hepatitis (AIH) and 35 cases of frozen tissue sections were stained with enzyme antibody indirect method. Results: The percentage of Vβ5.1 positive cells in liver infiltration was 18.9±12.3%(mean ± SD) for HC, 5.7±6.0% for HB and 1.3±1.2% for AIH(P<0.05). In PBC cases, 1 of 5 cases had Vβ5.1, and the positive rate was 66% and the positive rate was lower than 10%. Vβ5.1-positive cells are mainly located in the portal vein domain, and the distribution of V β 5.1-positive cells is similar to that of Leu2a-positive cells. On the other hand, the average positive rate of antibodies in other V domains was less than 5%. In addition, the positive rate of antibody in peripheral blood was analyzed (HC10 cases,HB5 cases,PBC7 cases,AIH5 cases, healthy 5 cases). The average rate of positive cells in various V domains was less than 5% in all diseases, and less than 5% in healthy people. In some cases of HC and PBC, Vβ5.1-positive cells were found to accumulate in the liver. The distribution of Vβ5.1-positive cells and UCHL1-positive cells was similar to that of Leu2a-positive cells, and the same cells were the host of memory cytolytic T lymphocytes. Now,PCR method is used in the discussion of the point, and it is scheduled to be published by the Japan Liver Society in Heisei 5 years.
