项目摘要

平成6年度までの、日本人肥大型心筋症患者を対象にした心筋βミオシン重鎖遺伝子のスクリーニングの結果、14種類のミスセンス変異と、1種類のナンセンス変異を見い出した。ミスセンス変異は、いずれも同遺伝子のHeadおよびHead-rod接合部にのみ認められ、rod部には1例の認められなかった。ミスセンス変異は家系内で疾患と連鎖していること、健常集団には認められないこと、いずれの変異も種を越えて保存されたアミノ酸に生じていることから、変異がこれらの家系での本症の原因と考えられた。しかし、エクソン3内に認められたナンセンス変異は、家系内で患者と一致しない事から原因とは考えられず、心筋βミオシン重鎖遺伝子の大きな構造変化を伴う異常は、心筋収縮要素に組み込まれず、本症の原因となり得ない事が明らかとなった。また、臨床病態の検討の結果、心筋βミオシン重鎖遺伝子異常のうちアミノ酸の電荷が変化するミスセンス変異を有しする症例群は、電荷の変化しない変異を有する症例群と比較して予後が悪く、心機能の低下も著明である事も明らかになった。これらの知見には、現時点でも臨床応用可能なものも含まれている。さらに80%以上を占める原因不明の症例群を対象とした研究も継続しており、日本人症例においてもαトロポミオシン遺伝子異常を有する症例が存在すること、しかしながらその頻度は極めて低いことも明らかにした。従って、現在残りの大半の原因不明の症例群を対象として、心筋収縮に関わる遺伝子群を中心に、心筋に発現している遺伝子群の構造解析を行っている。
