平成5年11月、宮城県下で開業している全歯科医院に対し、歯科医師会の協力を得て、障害者の歯科医療に関するアンケート調査を行った。その内容は障害者にどの様に対応しているか、どの様な点に困難を感じているか、将来どの様な対策を講じて行けばよいのか等であった。歯科医院院長宛に833通発送したところ、回答者数は414通で、回答率は49、7%であった。障害者の治療経験は、76、4%と高かったが、障害者の月平均来院数は、「10人以下」と一番低い選択肢を選んでいた医院が殆どであった。診療中、困難を感じた歯科医師は、73、5%に及び、出来る範囲の処置のみを行っている場合が多かった。診療を行った障害者の中で、困難を感じた障害として、精神発達遅滞の半数以上を挙げ、次いで、脳性麻痺、自閉症等であった。逆に、視覚、聴覚障害では少なかった。取扱法としては、通常の方法が殆どで、次に人手やタオルによる抑制が多かった。今後、障害者歯科医療を受け入れていくに当たり、スタッフ数、熟練度の問題、設備の不足、障害の程度、救急医療体制の問題等があった。障害者歯科医療の本来あるべき姿、将来像についてに関して、「一次医療機関、二次医療機関、三次医療機関の関係の確立し、県レベルでの障害者医療体制を作るべきである」、「国や地方行政機関が障害者専門のセンターを設立し、歯科医師会、麻酔専門医、障害者歯科専門医らがチームを作る事が望ましい」が多く挙げられた。東北大学附属病院障害者歯科治療部には、「教育機関として、障害者の身体的、精神的な問題を捉えるだけではなく、福祉に関する知識も、教育に取入れるべきである」、「障害者歯科を専攻した歯科医師をもっと多く養成してほしい」、「全身麻酔下治療を受け入れる体制を確立してほしい」等、多くの要望が寄せられた。今回のアンケート調査を踏まえて、宮城県の障害者医療を構築していきたい。
