项目摘要

潜在看護婦(就業意欲のある未就業看護職)の訪問看護への就業意欲と、その促進・阻害因子を把握することを目的として、広島県看護協会のナ-スバンク登録者を対象に調査を行った(配布987名・回収457名・回収率46.3%)。潜在看護婦の特性:得られたサンプルの年齢構成は、20代18.4%・30代58.9%・40代15.8%・50代4.6%・60代2.0%で、98.9%を女性が占めた。医療機関勤務経験者が9割弱、フルタイム勤務継続者が7割存在した。現在の平均離職期間は6.4年(標準偏差5.2年)で、「家庭要因(育児等)」を離職理由とする者が82.5%存在した。参加意欲:勤務形態に関わらす訪問看護を希望した者は46.4%で、パートタイムには40.9%、フルタイムには20.8%の希望があった。またパートタイムでは40.9%のうち15.5%が、フルタイムでは20.8%のうち9.8%が将来も継続する意志を示していた。パートタイムには、全体の約8割が希望、約4割が次年度には就業可能であり、量的即戦力としての意義が認められるが、その継続性はフルタイムより低く、雇用者側に管理スキルが要求される。促進・阻害因子:数量化理論第II類により、個人・家庭・職業因子39変数に対し変数選択を実施した結果、「看護協会主催の訪問看護講習会への意欲の有無」「訪問看護が好きな仕事か否か」「看護基礎教育過程が専門学校か否か」「乳幼児・小学生の有無」の順に高い寄与を示した。また「情報収集」と「アセスメント」の自己スキルも続く寄与率を示したが、これへの自信は訪問看護就業への阻害因子として作用していた。政策的課題:回収率を考慮すると、2割以上の潜在看護婦が好きな仕事として訪問看護への就業意欲をもっている可能性が高い。しかし自己スキルへの不安は大きく、約1ケ月間の訪問看護講習会を受講しようという前向きな姿勢にある。よって、訪問看護技術を獲得していくプロセスへの政策的支援体制が、マンパワー確保上の重要な課題として示唆された。
