项目摘要

1.内視鏡的吸引粘膜切除法は消化管疾患の診断・治療法として臨床的にきわめて有用であり、一定の条例を満たす早期胃癌野根治的治療法として広く普及している。2チャンネルの処置用内視鏡を用いる従来法では切除できる大きさは15mmが限度であり、熟練した術者によっても病変の部位によっては適応できなかった。これを改善するために内視鏡吸引粘膜切除法(以下EAM)を開発した。2.先端フードと内視鏡外側に装着するスネアガイドチューブを一体化した器具を作成し臨床応用を図ってきたが、平成7年8月には市販された。メーカーは当初500セットを用意したが、短期間に1000セットが全国の病院に納入されたと聞く。我々の学会での発表が注目されていたことが示された。3.本院での過去の手術症例の検討から、高分化型腺癌では20mmの大きさでもリンパ節転移がないことを見出し、この大きさまでは病変部位に関わらず絶対的治療の適応とし、52例の治療を行なった。長径25±5mm,短径21±5mmを切除出来た。高齢者や合併症がある症例では40〜50mmの病変も分割切除により相対的治療の対象と出来た。4.重篤な合併症として、初期に穿孔例、出血4例を経験し、最近では出血例3例が増えた。穿孔の予防のためには粘膜下に局注後に超音波内視鏡により粘膜下層の剥離を確認するようにし、以後経験していない。大きく切除できるため、出血例は増加しており、全国の施設からも出血の合併が報告されている。今後も改善のため努力するが、いずれも内視鏡的に止血しており、開腹に至った例はない。外科手術では全例出血することから、出血を恐れる必要はないと判断するに至っている。
