開瞼し探索している時の注視点の動きをアイマークレコーダを用いて調べることにより、精神分裂病に特有な異常がみられることが指摘されている。分裂病様状態の存在もしくは既住の認められた側頭葉てんかん患者16例と、精神症状のない側頭葉てんかん患者40例、分裂病患者41例、健常者42例を対象とし、このような探索眼球運動を比較した。その結果、注視点の運動数と総移動距離は分裂病群では28.2,437.1であり、他群(分裂病様てんかん群36.5,613.3、てんかん群39.2,676.1、健常者36.4,625.0)と比べ有意に少なかった。再認時の探索スコアーと反応的探索スコアーは、分裂病群では4.6,7.4であり、他群(分裂病様てんかん群5.9,10.0、てんかん群6.4,11.0、健常群6.2,10.4)と比べ有意に少なかった。すなわち、探索眼球運動の異常は分裂病群のみに認められ、分裂病様状態を呈する側頭葉てんかん患者は精神症状のない側頭葉てんかん患者や健常者と差がなかった。このことから側頭葉てんかん患者が、幻覚、妄想などの分裂病類似の状態を呈しても、その神経生理学的な病態は精神分裂病と異なることが示唆された。次にMRIを用いて海馬硬化を反映する所見を同定するため、脳幹に平行に5mmスライスの冠状断を行った。対象は側頭葉てんかん患者35例で、海馬萎縮の判定は視察的に、海馬体部の横経、縦経の縮小、海馬内部構造の消失、T1強調画像にて信号強度の減少、T2強調画像にて信号強度の増加を総合して行った。その結果、海馬萎縮のない17例、左側萎縮10例、右側萎縮4例、両側萎縮2例となった。その他、foreign tissue lesionが2例発見された。精神分裂病様状態を呈した側頭葉てんかん患者は5例みられたが、海馬萎縮がない例が3例、萎縮のある例が2例と差がなかった。すなわち、分裂病様状態の発現と海馬萎縮との関連はないと考えられた。
