我々は平成4年より胃癌とEpstein-Barr Virus(EVB)との関連性を研究しており、今年度新たに得られた知見は下記のとおりである。(1)EBV関連胃癌症例の臨床的特徴:EBV関連症例は全胃癌の4.9%を占めており、対照群に比し肝転移、腹膜播種が少ない傾向がみられたが、リンパ節転移率の差は認められなかった。(2)表面形質の解析:新鮮標本を用いた検索により、EBVを含む胃癌細胞では細胞接着分子(LFA-1,ICAM-1)やHLAクラスII抗原の増強する傾向が示された。しかしながら上記抗原発現が減弱している症例もみられた。(3)抗EBV抗体価の測定:EBV関連胃癌患者では諸種のEBV抗体価が上昇している傾向がみられたが、特に抗early antigen-IgG抗体価と抗viral capsid antigen-IgG抗体価上昇が特異的であり、腫瘍マーカーとして用いる可能性が示唆された。(4)浸潤リンパ球の検索:リンパ上皮腫型胃癌においては、浸潤リンパ球はTリンパ球が主体であるがリンパ球の増加とともにBリンパ球を主体とするリンパ濾胞の形成も認められる。Tリンパ球としてはCD4陽性リンパ球が有意であった。T細胞受容体(TcR)レハトアの検索は症例が少なく不十分であった。(5)HLAタイヒング:EBV関連胃癌症例数例に対しHLAタイヒングを行ったが、現在の症例数では統計学的処理は困難である。(6)EBVのlytic cycleの検索:EBV関連胃癌組織内でEBV産生を示唆する所見は得られていない。以上が今年度の結果であるが、今後とも症例数を増やし研究を継続していきたい。

The relationship between Epstein-Barr Virus(EVB) and gastric cancer was studied in the fourth year of Pingcheng. (1) Clinical characteristics of EBV-associated gastric cancer cases: EBV-associated cases accounted for 4.9% of total gastric cancer, and the proportion of EBV-associated cases was lower than that of liver metastasis and peritoneal seeding. (2)Analysis of surface characteristics: Fresh plum was used to detect the presence of EBV in gastric cancer cells, and there was a tendency to increase the presence of cell adhesion molecules (LFA-1,ICAM-1) and HLA II antigens. The presence of antigens in the brain is reduced. (3)Anti-EBV antibody assay: EBV-associated gastric cancer patients have a tendency to increase EBV antibodies, especially anti-early antigen-IgG antibodies, and anti-viral capsid antigen-IgG antibodies. (4)Infiltrational-type gastric cancer: Infiltrational-type gastric cancer: Infiltrational-type gastric cancer: Infiltrational-type gastric cancer T T-cell receptor (TcR) is the most common type of receptor. (5)HLA classification: It is difficult to carry out HLA classification of EBV-associated gastric cancer cases, but the current number of cases is difficult to handle statistically. (6) Detection of EBV lytic cycle:EBV production in EBV-associated gastric cancer tissues was detected. The above results are expected to be consistent this year, and the number of cases of mitosis will increase in the future.
