平成14年度に行った,研究を発展させ,その成果を報告した。従来法(音声評価,鼻咽腔造影,鼻咽腔ファイバー)により鼻咽腔閉鎖不全が判明している患者に対して,ナゾメータによる口腔,鼻腔の音響エネルギーの測定を行い,従来法による重症度判定と,ナゾメータによる重症度判定をretrospectiveに比較検討し,ナゾメータによるNasalance測定値と鼻咽腔閉鎖不全重症度との関連性,相関性について検討した結果,ナゾメータによるNasalance測定値は鼻咽腔閉鎖機能とよく相関することが確認された。新規の鼻咽腔閉鎖不全疑い患者に対して,ナゾメータと従来法を行い,両者でprospectiveに,検査の侵襲度,簡便性について検討した結果,ナゾメータによるNasalance測定は,従来法に比較して,簡便であり,検査による患者への侵襲が少ないことが確認された。ナゾメータと従来法,構音の主観的評価により,経時的に鼻咽腔閉鎖機能の改善度合いを比較検討し,鼻咽腔閉鎖機能ならびに構音の改善度と,ナゾメータによるNasalance測定値との関連性を検討したところ,鼻咽腔閉鎖機能の改善とナゾメータによるNasalance測定値の変化はよく相関していた。幼児を対象として検討したところ,ナゾメータは3歳以上の幼児において施行可能であることが確認された。視覚的フィードバックを用いた言語訓練を,構音障害を有する幼児に行ったところ,患児の興味を引き出すことができ,訓練の効率を高めることができた。以上の結果について,日本形成外科学会基礎学術集会,日本頭蓋顎顔面外科学会において,発表を行った。また,日本頭蓋学顔面外科学会誌上において,論文発表を行った。
