「看護は観察によって始まる」と云われる。そして視線の向こうに患者の顔がある。本研究は、心身一元論に立ち、患者の顔の表情を客観的に読み取り看護観察を科学とする試みである。昨年度に引き続きエクマンが云う、驚き、恐怖、怒り、嫌悪、悲しみ、幸福といった基本の感情の中から医療現場で遭遇する機会が多いと思われる恐怖、嫌悪、悲しみ、幸福の4つの感情を表す顔写真資料を259名のボランティアの協力を得て作成し、第3者による認知テストをクリアーした104名の額、眉、瞼、頬、鼻、唇、顎の位置情報を数値化し普通の表情との比較検討を行った。その結果、A.4つの表情について、以下の特徴を得た。これらは概ねエイクマンの説と一致するが、恐怖、嫌悪、幸福に関して異なる点があった。(1)恐怖:眉上る、目開く、口開く。驚きの表情と混同。(2)嫌悪:眉下る、下瞼の下にしわ、上唇上る。怒りの表情と混同。(3)悲しみ:眉下る、伏せ目、口角下る。(4)幸福:目細く、口開く、鼻から口唇端にしわ。B.次に、市販ビデオソフトを入手しAの結果を検証した。悲しみと幸福の表情に関してはAの特徴が確認されたが恐怖と驚きの表情、嫌悪と怒りの表情とを明確に区別する特徴を見いだせず、我々は連続した時間の流れの中で表情をとらえ判断している事が明らかとなった。C.最後に、患者の表情観察に移行したが、患者の顔写真を得る事が困難であり、且つ、看護者は患者の表情から心のうちを直ちに読み取り行動する必要がある。そこで、Aで得られた表情分析結果を本研究に協力してくれる看護師に知識として認識していただき看護業務現易で検証する事を始めた。此の件に関しては1年計画でデータを収集することとし本年1月より開始した。A、Bに関する研究結果を論文としてまとめつつあり、したがって裏面の研究発表欄は空欄です。
