项目摘要

平成14〜16年の「中高年女性の虚血性心疾患」に特異的な冠危険因子、ソーシャルサポート、QOLなど要因の調査研究、関連文献調査、および米国における本研究領域の看護研究者との交流、および循環器学会等における性差医療に関する研究データ・情報をもとに看護介入モデル構築に向けての考察をした。虚血性心疾患を持つ患者の病態生理・遺伝的、心理的、社会・環境的側面における要因において特異的な性差が認められた。発症率は有意に男性に高い。特に冠危険因子に関しては、男性では喫煙、肥満が、女性に特異的なものとして、耐糖能異常・糖尿病、高脂血症、家族歴が認められた。しかし、因子の重複による病態像などメタボリック症候群などで女性との関連性が注目される必要が有る。また、上述の生活習慣の虚血性心疾患発症に対するオッズ比の性差(特に喫煙は女性に約8倍も高い)、予後にみる重篤化や冠微小血管障害などにも介入上で重要である。ソーシャルサポート・QOLに対する低値、鬱状態などの精神心理的反応は女性に高く、セルフケア・自己管理行動改善に向けて性差の多義的な特徴を考慮にいれた中高年女性への看護介入のモデル構築が必要である。これらは、米国・本邦の介入上の課題動向としても注目されている。これらをもとに、システム論を活用して包括的な看護介入モデル構築(基礎的)を考察する:(1)中高年女性(特に閉経後)に特異的な先行要因の知識、(2)生活習慣・冠危険因子の軽減・改善への治療・支援の実践、(3)「最善となるQOL」達成に影響する要因理解・アウトカム指標の抽出である。性差の有無に関わらず本疾患の一次予防から三次予防による健康維持・増進・回復にむけての介入は不可決であるが、性差の視点を考慮に入れた介入モデルが必須であることが示唆された。更に情報の分析・チーム医療の中で看護職が担う役割について考察を深め、学術誌へ投稿すべく検討を継続する予定である。
