【目的】統合失調症患者の体力低下は、社会復帰を妨げている一因と考えられている。この点で、体力が社会復帰を判断する指標になりうる可能性がある。本研究では、精神症状との関連性から、統合失調症患者の精神的健康関連体力(mental health-related physical fitness)の概念を提案し、実データからその可否を検討し、それを測定するための簡易体力テストを構成することを目的とした。昨年度は、精神症状を外的基準とした基準関連妥当性の視点から、握力とステッピングの2項目からなる簡易体力テストを構成した。本年度は、社会生活における困難さを外的基準として、妥当性を評価した。【対象と方法】対象者は、精神科デイケア施設に通所し、研究への協力に同意した統合失調症患者男性32名である。体力テストは、血圧、身長、体重を測定した後、体脂肪率、背筋力、長座体前屈、上体起こし、ステッピング・タッピング・全身反応時間、ペグボードテスト、開眼・閉眼重心動揺の測定を実施した。精神症状は、精神科医らが5尺度40項目からなる精神障害者社会生活評価尺度(LASMI)により5段階評価した。抗精神病薬内服量はクロルプロマジン換算した。解析は、性別に年齢と抗精神病薬内服量の影響を除去した体力と精神症状の偏相関係数を算出した。【結果と考察】背筋力、ステッピング、ペグボードテストがLASMIの複数の下位尺度と有意な相関を示した。また、年齢、身長、体重、抗精神病薬内服量の影響を除去した体力と生活障害の偏相関係数からも同様な結果が得られた。これらの結果から、精神的健康関連体力の要素としては、筋力と敏捷性が妥当であることが再確認された。一方、巧緻性も重要であり、実用性からペグボードテストの代替可能性が示唆された。現在、縦断的観察に基づく回復経過との関連性の点から有用性を検討するため、データを収集している。
