项目摘要

この2年間の本研究で以下の事実が究明しえた。1.経産婦献血著6万人について抗血小板抗体をスクリ-ニングしたところ、(1)抗YuK^a抗体は6万人中76人(0.12%)に、YuK^b抗体は6万人中1人(0.02%)に検出された。これら77人の抗体は全てIgG抗体で、これらの人々には輸血暦のないことから母児間のYuK型不適合妊娠により産生された抗体であることが判明した。これらの人々の分娩暦より新生児血小板減少性紫斑病の有無を明確にしえなかったが、本邦では今回のスクリ-ニング検査の結果から、YuK^a不適合が新生児血小板減少性紫斑病の主要な原因であることの論拠を得た。(2)抗YuK^a抗体保有経産婦献血者22人の協力を得てHLAタイピングを実施したところ、その内の16人(72.6%)がA24-Bw52-DR2のハプロタイプを持っていた。本邦でのこのハプロタイプの頻度は約9%なので、抗体保有者で有意にこのハプロタイプの頻度が高いことが判明した。このことは、今後の本疾患の予知、予防に重要と思われた。(3)経産婦献血者3万人中抗Sib^a抗体保有者1人、同献血者6万人中抗NaK^a抗体保有者2人を検出した。2.脳内出血を起こした重度の新生児血小板減少性紫斑病症例の母親血清の分析を実施し、抗体BaK^a抗体を同定した。これは本邦で、脳内出血を来した本疾患で特異性が解明された初めての症例である。周産期での児の脳出血には本疾患が関与している例は相当数あると推定されるので、本疾患の予知、予防は重要な課題である。3.血小板に対するマウスモノクロ-ナル抗体11種類を分析したところ1種類(OKM5)に抗NaK^a抗体の特異性があることを突き止めた。血小板型に対するモノクロ-ナル抗体としては初めてのものである。

During the two years of this study, the following events were investigated. 1. Antiplatelet antibodies were detected in 60,000 pregnant women who donated blood.(1) Anti-YuK ^a antibodies were detected in 76 out of 60,000 (0.12%). YuK^b antibodies were detected in 1 out of 60,000 (0.02%). It was determined that the antibodies of these 77 people were all IgG antibodies, and these people were not able to receive blood transfusions. However, it was determined that the YuK type in the mother and child was not suitable for pregnancy and produced antibodies. The results of this study are not suitable for the main causes of neonatal thrombocytopenia purpura. (2)Anti-YuK ^a antibodies were retained in 22 maternal blood donors, and 16 (72.6%) of them (A24-BW-52-DR-2) maintained their HLA status. In this country, the frequency of anti-virus infection was about 9%, and the frequency of anti-virus infection was higher than that of anti-virus infection This is an important thought for the future to know and prevent this disease. (3)One anti-Sib ^a antibody holder out of 30,000 maternal donors and two anti-NaK ^a antibody holders out of 60,000 maternal donors were detected. 2. Analysis of maternal serum from severe cases of neonatal thrombocytopenia purple spot due to onset of internal bleeding and determination of BaK^a antibody The disease is specific to the patient and causes internal bleeding. It is important to know and prevent this disease. 3. The specificity of anti-NaK ^a antibody was analyzed by using the method of 11 kinds of anti-NaK ^a antibody (OKM). Platelet types are not immune to infection.
