项目摘要

医療の高度化と先端技術への依存の増大は、医療従事者としての看護領域においても同様に知識の体系化・養成制度改革、技術の高度化・専門分化をひきおこしている。一方では集中管理室や透折看護に象徴されるように、先端技術を介する高度技術的な患者との対応が生じており、そうした知識・技術を身につけた看護が要請されている。地方ではこの様な動きを「小型の医師」「医師モデルの看護」として排し、逆に旧来からの対人関係中心の看護、対人的ケアのもつ意義が「看護の本質」「看護のこころ」として見直され深化されている。新たな技術の不可避的傾向にあって、後者(本質、こころ)を踏まえた前者への対応という視点が確認されるべきであろう。それは次の課題とも密接に関連する。21世紀の高齢化社会に向けた医療・保健・福祉の総合化されたシステムの問題である。岩手県沢内病院の場合、深沢村長という優れた一政治家の僻地医療に結合した村の健康行政があったし、長野県佐久病院若月院長の場合も、農村医療の40年余にわたる実践活動の蓄積が今に生きている。これは共に、そこに生活する人々の健康が大事なのであって、病院と医療従事者はそのためにあるのだという当然の前提から出発し、それを支える地域と医療のシステムと運動を不断に再生させることに成功したケ-スということができる。現状は決して楽観を許さないものがある。「自分たちの健康を自分たちの手で守る」為には、行政、病院そして地域住民が一体となったシステムをその地域にふさわしいやり方で形成していく必要がある。看護の専門職性と職業意識の形成も、決して一病院内で完結し得ないものであることがわかる。自然・社会・文化の環境をも視野に入れた地域独自の意識的な取り組みとの接点をもつこと、その地域の生活・歴史・文化遺産の中に生きる専門職性と職業意識、先の二つの先進事例はその重要性を今のわれわれに教えている。
