项目摘要

リンパ増殖性病変にうち本年度はAILDと同様にT細胞増殖疾患であり、病変が極めて反応性性格を呈しながら予後不良病変である2つの型、即ちT領域性リンパ腫TzLと類上皮細胞性リンパ肉芽腫(リンパ類上皮細胞性リンパ腫)LeLについて、前年度と同様の方法により二重免疫組織化学的手法及びTCRとIg遺伝子再構成の検出法を行い、それらにおける"真の"増殖細胞のPhenotypeと増殖細胞のclonalityの存否を検索した。その結果、1、この2型の病変における"真の"増殖細胞は殆どの例でCD4thelper/inducer T-cellであり、特にLeLでは純粋にCD4+Cellの増殖である。2、TzLでは14例中8例に、LeLでは9例中5例にTCR_β遺伝子のclonalな再構成を認めこの率はAILDのそれと同程度であった。更にTzLで組織像中に濾泡を有するものと、ないものとではそれぞれ4/10、4/4と検出率に差がみられたが、これはAILDにおける組織学的異型度による検出比率の差と同様に考えられる。3、「濾泡過形成を伴なうT領域異形成」と我々が名づけた病変について同様の検索を行い、1)これらがCD4+helper T-cellの増殖病変であること。2)9例中3例にTCR_β遺伝子の再構成を認め、特に1例で病変から採ったT-cell-rich分画にのみ再構成を証明した。このことは病変中に少なくはあるがclonal成分が存在することを示すものである。これらの所見より、これらT細胞増殖病変はその反応性様相にも拘らず、恐らくすべての病変中にclonalに増殖するpopulationを含むもので、その意味で"不完全ながら"腫瘍性病変と考えられた。

In addition, there is a difference in the ratio of 4/10 and 4/4 of the follicle in the tissue image. 3."Follicle formation is accompanied by T domain differentiation." 1) CD4 +helper T-cell proliferation is accompanied by T domain differentiation. 2)9 Three cases of TCR_β gene recombination were identified, and one case of TCR_β gene recombination was proved. This is the first time I've ever seen one. The results of this study indicate that T cell proliferation is not a complete disease.
