项目摘要

前頭葉及質機能を反映するWisconsin Card Sorting Test(WCST)の原法は分類枚数が148枚と多く注意集中持続が困難なケ-スがあるなどの問題があるとされ、いくつかの修正法が提出されている。また、カ-ド提示から分類遂行までの反応時間の測定ができない。これらの問題を解決するため電算機プログラムを用い短縮したWCSTを開発した(CーWCST)。CーWCSTは電算機画面で分類を行う新しい方法であるため、テスト施行時の教示法を決定するため、健常成人6名、分裂病者2名に施行を繰り返し施行方法の検討を行った。その結果、一度の検査ではCーWCSTを2回連続し、1回目は分類方法の教示は行わず2回目には、色形数の分類カテゴリ-があることと、正答が続くと自動的にカテゴリ-の変換があることを教示する方法が妥当であると判断した。評価項目として、達成カテゴリ-数、総誤解答数、保続数、カテゴリ-変換時の試行回数と、連続する正解答10個のうち後半7個に要した反応時間の平均(R),カテゴリ-変換時の試行に要する反応時間(T)、保続解答時の反応時間(P)の3つの反応時間を測定することにした。分裂病者では健常者に比較し、総誤解答数、保続数が増加していることが示された。この分裂病者での前頭葉機能障害の所見はWCST原法を用いた分裂病者の所見と同一のものであった。反応時間は、健常者のR:1.04秒,T:2.08秒,P:1.87秒に対し、分裂病者のR:3.25秒,T:7.19秒,P:4.78秒と、いずれも分裂病者で延長していた。現在、健常者の例数をふやし、CーWCST法での正常範囲の設定を行うとともに、ド-パミン神経遮断スルピリドの投与によって評価項目の各値と、反応時間にどのような変化が生ずるかの検討を開始している。
