HRP(horseradish peroxidase)の血管外漏出は再潅流後30分、48時間後に認められ、再潅流3時間、6時間、24時間後には認められなかった。形態学的には、再潅流30分後ではpinocytotic transportの亢進による血液脳関門(BBB)の透過性の亢進が、48時間後ではtight junctionの破綻によるBBBの透過性亢進が観察された。以上より、心停止蘇生後のような完全全脳虚血後においても、不完全脳虚血で報告されているような二相性のBBB透過性変化が認められ、再潅流後早期のBBB透過性亢進は可逆的であり、その透過性亢進には再潅流時の反応性充血が大きく関与していることが示唆された。また、48時間後のBBB透過性亢進は非可逆的であり、血管内皮細胞自体の虚血性変化によるものと考えられた。従って、心停止蘇生後早期には正常状態ではBBBを通過しがたいとされる薬物も容易に脳内に移行し、脳外作用しか有しない薬物でも脳機能に対して予想外の薬理作用を及ぼすことが示唆された。

The extravascular leakage of horseradish peroxidase (HRP) was detected 30 minutes, 48 hours after reperfusion, 3 hours, 6 hours and 24 hours after reperfusion. Morphological examination showed that after 30 minutes of reperfusion, the permeability of BBB was increased, and after 48 minutes, the permeability of BBB was increased. After cardiac arrest, the BBB permeability of the two phases is increased, and the BBB permeability is increased in the early stage after reperfusion. However, the BBB permeability is increased in the reversible stage. After 48 hours, the BBB permeability is increased, and the vascular endothelial cells are spontaneously reduced. In the early stage after birth, BBB is easy to move inside and outside the body, and its function is to move outside the body.
