平成4,5年度にわたり,慢性疾患患者のホームケアにおける障害度別にみたケアに関わる費用分析を実施し以下の結果を得たので報告する。対象は,東京女子医科大学病院の脳神経外科病棟3個所および神経内科病棟2個所を1年間に生存退院した20歳以上の患者とその家族450ケースである。障害度の分類はKATZのADL-INDEXをもとに新たなADL尺度を開発した。その尺度は歩行,入浴,更衣,会話,排泄,食事の6アイテムで,各々5-7項目のサブスケールで構成され,因子分析とガットマン解析により,尺度の1次元性と再現性が確認され,障害度は8段階に分類された。一方,費用は直接医療関連支出と間接療養関連支出とに分けた。後者は在宅におけるケア時間を測定し,それを各職種の賃金体系をもとに便宜的に費用に換残した。その結果,直接医療支出は障害度が高くなっても漸増を示した。このことは,障害度が高くなるにつれて病院外来受診が減少することを反映している。一方,間接療養支出は障害度が高くなるにつれて指数曲線的に増加する傾向を示し,特に,排泄や食事といったADLが障害される段階まで進んだ対象では,急激に間接療養支出が高まることが示された。しかし,障害度が高い対象では間接療養支出の分散が大きく,今後,障害度が高い群の費用支出に関する測定精度を高めることが課題である。これからの方向性として,米国のナーシングホームの費用償還に活用されているRUG-SYSTEMを我が国のホームケアに適用することの検討を計画している。

In 2004 and 2005, the cost analysis of chronic disease patients was carried out and the following results were reported. For example, 3 neurosurgical departments and 2 neurosurgical departments of Tokyo Women's Medical University Hospital survived for 1 year, and patients aged over 20 years were discharged from the hospital. The classification of impairment is based on the ADL-INDEX of KATZ. The scale of walking, bathing, changing clothes, conversation, excretion, food, each of the 5-7 items of the structure, factor analysis, scale of 1-dimensional reproducibility, impairment classification, 8-level classification. On the one hand, expenses are divided into direct medical-related expenses and indirect recuperation-related expenses. The latter is determined by the cost of each job. As a result, direct health care spending is increasing. This is reflected in the decrease in the number of visits from outside the hospital. On the one hand, indirect recuperation expenditure has a tendency to increase due to the high level of impairment, especially due to the high level of impairment due to excretion and ADL, and on the other hand, due to the high level of acute indirect recuperation expenditure. In the future, there will be a high degree of impairment and a high degree of accuracy in the measurement of indirect recuperation expenses. This is the first time that the RUG-SYSTEM has been applied to China's infrastructure.
