1.夜間訪問実態把握のための介護報酬分析研究東京都内3自治体の3ヶ月分の介護報酬を得て、夜間訪問介護を利用している利用者の要介護度分布、性・年齢、夜間訪問介護サービス内容別、訪問時間の長さ別、夜間訪問介護を利用している利用者の日中時間帯の訪問介護利用実態についても自治体別に分析を行った。A自治体のある月の結果は、訪問介護利用者3537人。内、早朝夜間訪問介護利用者194人。深夜訪問介護利用者28人であった。ある月の1ヶ月延べ訪問回数をみると、早朝夜間帯は2572回。深夜帯は1017回であった。3ヶ月のデータから比率を換算すると、訪問介護利用者を100人とすると、早朝夜間帯の訪問介護利用者は5.47〜5.51人、深夜帯の訪問介護利用者は、0.79〜0.82人であった。要介護度分布をみると、要支援者も2%前後と少ないものの夜間訪問介護を利用している実態があり、要介護1・2が約25%、要介護3が約20%、要介護4・5が約50%である。年齢は50歳代の利用も数人あり、85歳以上の利用が約半数を占めている。夜間帯の訪問介護を利用している人は、日中、平均2・2種類の訪問介護を利用している。2人介助訪問介護利用もあり、重介護状態が推測された。一方、長時間の生活援助利用実態もある。2.訪問介護事業所の夜間訪問体制と夜間訪問介護利用事例ヒアリング調査ヒアリング調査を2事業所で行った。夜間訪問介護利用事例の経年変化、現在夜間訪問を利用している事例の訪問介護計画、事業所の夜間訪問体制について情報を得た。3.潜在的な夜間訪問需要推計モデル作成と検討に関する研究1)平成15年度・16年度研究で得た要因を、マクロレベル、メゾレベル、ミクロレベルの要因カードに整理し、ハンドシュミュレーションでカードを動かしながら、「訪問介護の潜在的な夜間訪問需要推計のためのモデル案」を複数パータンを考え、「訪問介護の潜在的な夜間訪問需要推計のためのモデル案」を作成した。
