项目摘要

The objectives of this study were to clarify the actual status of interactions performed at home between demented elders and their family caregivers, and to develop the interaction model between them. An inductive qualitative study was conducted by using a grounded theory approach.Based on a constant comparative analysis, results revealed the levels of "certainty" of the elders as core variables in interactions between the elders and the caregivers. Characteristic interactions were found to fall into three categories. The category 1 is the interactions of caregivers' persistence of "certainty" of demented elders, category 2 is the interactions when the "nucertainty" of demented elders is unquestionable, and category 3 is interactions when "certainty" of demented elders is clear. In the category 1, subcategories named "looking for the holes", "digging the holes" and "filling in the holes" according to the caregivers persistence of "uncertainty" of demented elders were found. In the category 2, phenomena named "protection", "friction", "harmony" and "resistance" were observed. And the category 3 contained "equality".Premise of the interaction model is that the interactions between the caregivers and demented elders are largely affected by the manners of attitude of caregivers. There are 2 points in the general way of interactions. One point is the caregivers should pay attention not only to the "dementia" of the disease but also to their expressions of healthiness and personality. The following are to be considered : 1.to interact not to lower demented elders' existence, 2.to make effort to facilitatethe potential of demented elders for taking the attitude of others. The other point is that the caregivers and demented elders have various interactions. It is important that the actual way of interaction with demented elders is the diversity of interactions.
本研究旨在了解失智症老人与其家庭照顾者在家中互动的现况,并建立失智症老人与其家庭照顾者之间的互动模式。本研究采用扎根理论方法进行归纳性质性研究,通过持续的比较分析,揭示了老人的“确定性”水平是老人与照顾者之间互动的核心变量。特征相互作用被发现分为三类。第1类是照顾者对痴呆老人“确定性”的持续性的交互作用,第2类是痴呆老人“确定性”不可质疑时的交互作用,第3类是痴呆老人“确定性”明确时的交互作用。在第1类中,根据照顾者对失智老人“不确定性”的坚持程度,分别出现了“找洞”、“挖洞”和“填洞”的亚类;在第2类中,分别出现了“保护”、“摩擦”、“和谐”和“抵抗”的现象。第三类为平等。本研究假设照顾者与失智老人之间的互动关系在很大程度上受到照顾者态度方式的影响。一般的互动方式有两点。其中一点是照顾者不仅要注意疾病的“痴呆”,而且要注意他们的健康和个性的表达。以下是需要考虑的:1.to不要降低痴呆老人的存在,2.to促进痴呆老人采取他人态度的潜力。另一点是,照顾者和痴呆老人有各种各样的互动。重要的是,与痴呆症老人的实际互动方式是互动的多样性。
