急性心筋梗塞症例において冠動脈造影後、再疎通療法(PTCRあいるはPTCA)を行った29例(42-84才、平均年齢62才、M:F=23:6)を対象にタイプIIIコラーゲン再生の指標である血中PIIIP、及びタイプIVコラーゲン再生の指標IV型コラーゲン7Sを経時的に(再疎通前、直後、2、3、5、10日より5日おきに30日まで)測定した。梗塞サイズが大きいほど、心筋の線維化も活発に起こると考えられたため、梗塞サイズの指標として、peakCPK、peakCK-MBとpeakPIIIPとの関連について検討したが、いずれのparameterとも有意な相関関係は認めなかった。また、心筋梗塞発症から再疎通までの時間が長いほど壊死心筋すなわち梗塞巣が大きいと考えられたため、再疎通までの時間とpeakPIIIPとの関連性についても検討したが相関関係は得られなかった。さらに、発症から再疎通までの時間と、PIIIPがpeakとなるまでの日数の間の関連性についても相関関係は認めなかった。PIIIPとpeakCPK、peakCK-MBとの間に相関関係が見られなかった原因の一つとして、急性期にPTCR、PTCAなどの再灌流療法を行った症例が多いため、peakCPK、peakCK-MBが必ずしも梗塞サイズを反映していない可能性が示唆された。次に心室瘤が認められた症例では治癒過程が遅延している可能性を検討するため、下壁梗塞を除いた20例において、心室瘤の有無とPIIIPがPeakに達するまでの日数について関連性を検討した。その結果、慢性期に心室瘤を認めた症例ではPIIIPがPeakに達するまでの日数が有意に延長していた。これは、慢性期心室瘤のある症例では心室線維化過程になんらかの問題があり、peakが遅れるために壊死巣の線維化が遅延し心室瘤が形成された可能性を示唆するものと考えられた。

PTCR was performed in 29 patients with acute myocardial infarction (42-84 days, mean age 62 days, M:F=23:6) after coronary angiography and PTCA. The index of regeneration of type III and IV was measured before, after, 2, 3, 5, 10 days after PTCR and PTCA. The correlation between the parameter of infarction, peak CPK, peak CK-MB and peak PIIIP is significant. The correlation between the time of onset of myocardial infarction and the time of reopening of myocardial infarction is discussed.
