右心バイパス手術(Fontan手術)後の血行動態・肺循環に及ぼす 1)大量アムリノン投与 2)低体温管理 3)内皮非依存性血管拡張因子である一酸化窒素(NO)ガス吸入 の効果を実験的に検討した。方法:雑種成犬を対象にPentobarbital麻酔人工呼吸器による調節呼吸下に胸骨正中切開心臓を露出,大動脈・肺動脈・上大静脈・左心房に圧カテーテル・大動脈に心拍出量測定用flowmeter装着。同種大動脈ホモグラフトを右心房・主肺動脈間に吻合し,主肺動脈を遮断・三尖弁を閉鎖して完全右心バイパス(Fontanモデル)を作成した。各種CVP条件下に心拍出量・肺血管抵抗(PVR)を測定コントロールとした後,1)〜3)の効果を検討した。結果:右心バイパス後心拍出量はCVPと有意の正の相関をもって上昇,正常心拍出量の維持にCVP17mmHg以上を要した。アムリノン5〜20μg/min/kg投与によりdose-dependentにPVR低下・心拍出量増加を認め,NO吸入5〜10ppmにてその効果は更に増強する傾向を示した。一方低体温管理は若干のPVR低下を示すものの,その効果は軽度であった。結論:アムリノン投与・NO吸入の有用性が示唆され,今後臨床応用が可能と考えられた。

Hemodynamics after right heart surgery (Fontan surgery)·Pulmonary circulation and hypothermia 1) Mass release and 2) Hypothermia management 3) Endothelium-independent vasodilation factor, NO, inhalation and effect evaluation Methods: In adult dogs, Pentobarbital anesthesia and artificial respirator were used to expose the heart through median sternotomy under regulated respiration, and the arterial, pulmonary, superior vena cava, left atrium pressure and cardiac output were measured by flowmeter. The aorta is closed to the right atrium, the main pulmonary artery is closed to the right atrium, and the main pulmonary artery is closed to the right atrium. Cardiac output and pulmonary vascular resistance (PVR) were measured under various CVP conditions. Results: CVP and positive correlation increased significantly after right heart failure, and CVP was significantly higher than 17mmHg during normal heart failure. The dose-dependent PVR decreased and cardiac output increased when NO was inhaled at 5 ~ 20μg/min/kg, and the effect increased when NO was inhaled at 5 ~ 10ppm. A hypothermia management system is designed to reduce the risk of PVR. Conclusion: The usefulness of NO inhalation in the treatment of acute respiratory failure is demonstrated, and clinical use may be investigated in the future.
