医科領域で大きな問題となっている遷延性術後疼痛について、歯科口腔外科領域では、極めて希であり、その大きな原因として通常局所麻酔薬が併用されるためと考えられる。今回局所浸潤麻酔・硬膜外麻酔を使用しない消化器外科領域における腹部手術、耳鼻科領域における上顎洞根治術、整形外科領域での頸椎前方固定術における腸骨採骨術と、手術の際局所麻酔を使用する口腔外科領域の手術について術後遷延性疼痛を訴えていた症例の頻度に関し比較検討してみた。その結果口腔外科領域での術後遷延性疼痛は3000例中1例であり、その1例は下顎骨骨髄炎で局所掻爬を行った症例であり、根治的に三叉神経第III枝のの捻除術を行った結果疼痛は消失した。またいわゆる医科領域での遷延性術後疼痛は約20000例17例に認められ、局所麻酔を施行しなかった症例に比べて優位に多く、統計的にも有意差が認められた。医科領域の遷延性術後疼痛の症例数は医学的ペインクリニックを受診した症例に限ったため自発性の有無によったが、医科領域での遷延性術後疼痛の症例数は潜在症例を数えるとより多数になるものと思われることから、局所麻酔の使用の有無は術後遷延性疼痛の発症に関して大きな因子となり、全身麻酔時に局所麻酔を併用することは予防方法として統計的にも臨床面でも有効な方法であると思われた。また、前投薬その他に遷延性術後疼痛を惹起するような著名な因子の差違は認められなかった。
