项目摘要

日本の超高齢化社会に伴い，認知症を合併した入院患者は急増傾向にあることから，臨床現場では，主疾患だけでなく認知症へのケアも求められている．しかし，認知症者特有の行動・心理症状（BPSD）のケアに困難感を抱くスタッフは少なくない．特に本人の生活歴や趣味・興味，思いを理解できないといったコミュニケーションに苦慮を示している．これまで認知症高齢者の生活歴や興味を引き出すコミュニケーションtoolとして色カルタを試行し，認知症者のコミュニケーションを促進させ，BPSDの低減，介護負担の改善に役立つことを明らかにした．本研究の目的は，これらの成果を発展させ，体系的なプログラムの構築および実践マニュアルを開発することである．今年度は，色カルタの使用経験のあるものをメンバーとした検討委員会による暫定版実施マニュアルの検討を行うための資料として，これまでに撮りためた色カルタを活用した実施場面の様子の動画分析を進めた．色カルタ実施中の参加者同士の交流場面に関して，回数を重ねるごとに参加者の発言・発信回数が増え，会話の内容は発話相手の背景を踏まえたものに変化してくことが確認できたたことから，参加者同士の会話の特徴について単グループの分析を行った．その結果，参加者同士の会話には〔参加者が自ら他の参加者に対してアクションを起こす〕，〔リーダーと参加者の会話に他の参加者が参加する〕，〔リーダーが参加者間の会話を中継する〕の〔3の局面〕ならびに＜9つの特徴＞が明らかとなった．今後は，複数グループの分析を通して，参加者同士の会話の特徴を明らかにするとともに，検討委員会での協議を併せて暫定版実施マニュアルを作成する予定である．
