経済的に困窮した人ほど健康を害しやすいが、必要な医療受診を控えやすい。困窮する患者の医療受診を経済的に支援する無料低額診療事業（無低診）は、医療機関が定める基準を満たした患者の医療費の窓口負担を減免できる社会福祉制度である。本研究では、無低診利用者の全国的な実態を明らかにするために、無低診の利用者データの全国レジストリを構築することを目的とした。2022年度4月からは研究参加機関の利用者のデータがレジストリへと登録され、約50実施医療機関が研究参加し、およそ350名のデータが収集された。収集された無低診レジストリ研究の登録データからは、利用者の約４割に過去１年間の経済的な理由による受診控えの経験があり、無低診の認知度によっても受診控えが緩和されないこと、利用者のうちの約半数は、今までに生活の困難を誰にも相談しておらず、無低診の利用をきっかけに始まる社会福祉による支援があることが示唆された。あわせて、医療ソーシャルワーカーによる無低診の実践を共有する場をオンライン形式で開催した。これらの内容を報告する場として、無低診研究フォーラムを開催し、参加者とともに無低診の可能性や課題を共有した。無低診の全国的な課題を解決していくためには、本研究が今後も多くの日本各地の医療機関と協力し利用者の状況把握をさらに進め、困窮する人々が無低診を利用するための障壁の除去に貢献できる資料を作成していくことが求められる。無低診により減免した医療費と税制上のインセンティブを考慮した無低診の経営影響を記述することが求められる。最後に、無低診の現代的な社会保障制度の中での役割や位置付けに関するさらなる議論が重要である。
