项目摘要

日本における若年女性は、強いやせ願望を有することが多くの研究で報告されており、近年問題視される隠れ肥満の基盤となっていることが推察される。また、BMI正常例において体脂肪率が健康障害に及ぼす影響について検討した報告は少ない。昨年度は、非肥満若年成人女性における隠れ肥満が及ぼす病的意義について、耐糖能、脂肪肝、食行動の観点から検討した。BMI 25kg/m2未満の20代女性60名を対象とし、体組成（身長、体重、体脂肪率、筋肉量、骨格筋量、骨格筋指数、腹囲、ヒップ径）、握力、血液検査項目（HbA1c、インスリン、アディポネクチン、遊離脂肪酸、LDLコレステロール、HDLコレステロール、中性脂肪、血小板数、AST、ALT、γ-GTP）、75g経口ブドウ糖負荷試験（OGTT）、食行動（BDHQ、EAT-26）を評価した。隠れ肥満における体脂肪率の明確なカットオフ値は定義されていないことから、体脂肪率の中央値で対象者を2分し、群間比較を実施した。対象者のうち、14名（23.3％）にOGTT120分値≧140mg/dl、27名（45％）にOGTT60分値≧155mg/dlを認めた。体脂肪率高群は低群に比し、HOMA-IR、γ-GTP、脂肪肝指数、中性脂肪が有意に高値を示し、ASTは有意に低値を示した。また、炭水化物エネルギー比率は有意に低値を、脂質エネルギー比率及びEAT-26のダイエット尺度項目は有意に高値を示した。以上より、非肥満若年成人女性においても、体脂肪の蓄積は、脂肪肝のリスクやインスリン抵抗性を惹起し、耐糖能障害、ひいては2型糖尿病を招く可能性が考えられた。さらに、これらの背景として、食行動異常の可能性が示唆された。
