项目摘要

看護学生の実習での看護行為を分析し、患者の安全の視点から問題に気づくというリスクセンスを育成することを目的としている。2022年度は、医学中央雑誌Webを用いて文献を検索した。リスクセンスでは文献が少ないことから、「医療安全」の下位概念となる「患者の安全／TH」と「看護学生／TH」による文献をかけ合わせて検索した。その結果、学生が危険と感じた場面および実際の経験では、「転倒・転落」「移乗・移送」が多いことが明らかとなった（小林,2020：仲下・河野,2016）。さらに、過去5年間の基礎看護学実習Ⅱにおける学生のインシデント・アクシデントを分析したMikiko H.et al（2016）は、移乗の場面が多いことの理由に、学生自身が危険を予測する能力の不足を指摘していた。これらの結果を得て、インタビューをするための質問項目を検討した。主な内容は①「ヒヤッ」としたり「ハッ」としたりした場面の発生状況、②患者に実施していた援助項目、③患者の状態である。経験型実習教育の重要性を提唱している安酸（2015）は、学生の実習中の直接的経験の意味を深化させることが学びを深めると述べている。これにより、インタビューを実施する際の教師としてのかかわりの注意点を検討した。主な方法は、直接的経験の意味を探求し、反省的経験に深化させていくプロセスに教師が対話を通して関わることである。以上より、看護学生の実習での「ヒヤッ」としたり「ハッ」としたりした場面の看護行為を分析することは、患者の安全を守ることのみならず、学生に対してもその意味を深化させることへつながるため、意義があるといえる。
