Apple Watchを用いた本研究であるが、Apple Watchの添付文書上の警告である「本品の通知結果を自己解釈し、医師の診断なしに、服薬中の薬剤の変更や中止等をおこなわないでください。」および「本品は心房細動を示唆する不規則な心拍を常時監視するものではないため、この機能を常時モニタとして使用しないで下さい。」に該当する使用方法であり、適応外使用として特定臨床研究に該当することとなり、特定臨床研究として認定臨床研究審査委員会の承認を得られるように安全性を重視したプロトコルへの変更を行った。安全性への配慮のため、選択基準の脳梗塞リスクスコアであるCHA2DS2VAScスコアを当初の4点以下から3点以下へと変更を行った。このことにより、対象患者が心房細動アブレーション術後の抗凝固療法については明確な指針がなく主治医の判断に委ねられていた症例になることで、Apple Watchを用いることでより安全に抗凝固薬の中止・継続が判断できることとなった。また、被験者のプロトコル逸脱の防止のため、抗凝固薬内服再開の基準は、従来の不規則な心拍の通知または高心拍数の通知を経てApple Watchでの心電図診断による心房細動の有無で行う方針としていたが、不規則な心拍の通知または高心拍数の通知が出現した場合へと変更した。洞復帰した後の再度の抗凝固薬の中止は、従来30日間心房細動を認めなければ中止としていたが、行わないことへと変更した。これらの変更により、より遵守しやすいプロトコルとなり安全性が確保されると考えられる。心房細動の有無に従い、抗凝固薬を内服・中止とする方法は、本研究で安全性が証明された後、次の研究で検証する予定である。
