项目摘要

病院が採用した科学的根拠に基づく実践（Evidence-based practice：以下、EBP）は、必ずしも継続的に実施されず、形骸的に実施される場合もある。病院が導入したEBPを看護職一人ひとりが継続的に実施するための取り組みや支援が必要である。その一つとして、部署の組織学習活動（EBPの継続につながる集団の学習活動）があり、病院の各部署が、組織学習活動を行うことで、部署の看護職のEBPの内在化（部署全体でEBPの根拠と意義を理解し、肯定的態度をもって実践に取り組んでいる状態）が高まり、看護職のEBPの継続的な実施につながる可能性がある。本研究は、病院が採用したEBPに対する部署の組織学習活動を高める/低下させる要因と条件を特定し、EBP継続のための部署の組織学習活動への影響を検証することを目的とする。本研究により、EBPを行う部署が組織学習活動を行うための諸条件が明らかとなり、活動促進に向けた実装戦略構築や教育プログラム開発に寄与できる。初年度は、調査1として、病院が採用したEBPについて各部署が行う組織学習活動を高める/低下させる要因の抽出を目的として、病院が採用したEBPを実施する部署の看護職（看護管理者と看護職）を対象に、オンラインのグループインタビューを実施した。インタビューで尋ねるEBPは部署ごとに事前に選定し、組織学習活動の対象EBPとした。インタビューは、インタビューガイドに沿って、対象EBPに対する部署の組織学習活動を高める要因や阻害する要因、必要な資源や支援等を尋ねた。また参加者には、アンケートにより自身が認識する自部署の組織学習活動の程度を測る集団の組織学習活動尺度と個人属性を尋ねた。本研究に関連して、集団の組織学習活動尺度の開発、集団の組織学習活動とEBPの継続的な実施に対するEBPの内在化の媒介メカニズムについて国内学会と国際学会で発表した。

Evidence-based practice (EBP) is used in hospitals, but it must be carried out in a scientific way. The hospital has a nursing staff and a support team. The possibility of organizing learning activities (EBP training activities) for each hospital deployment, organizing learning activities, and internalizing EBP for the nursing position (understanding the root meaning of EBP training for all deployments, affirming the attitude, and organizing learning activities) is high, and the possibility of implementing EBP training for nursing positions is high. The purpose of this study is to investigate the impact of EBP on organizational learning activities deployed in hospitals. This study aims to develop and deploy the conditions for organizing learning activities. In the first year of the survey, the hospital adopted EBP, and each deployment organized learning activities for high/low reasons. The hospital adopted EBP, and the deployment of nursing positions (nursing managers and nursing positions) was carried out. EBP is deployed in advance to organize learning activities. To find out the reasons, necessary resources and support for the organizational learning activities deployed along the route and target EBP. The participants were able to identify the extent to which they had self-deployed organizational learning activities and to identify the individual attributes of the collective organizational learning activities. This study is related to the development of organizational learning scale, the implementation of organizational learning activities and the internalization of EBP in Chinese and international societies.
