研究課題名「終末期ではない高齢透析患者がACPについて考えたきっかけ」の進捗状況は、研究計画書作成を行い、2023年4月14日に武庫川女子大学研究倫理委員会の研究倫理審査を申請したところである。研究倫理審査委は5月27日開催予定である。本研究の目的である、医療者が高齢透析患者へのACP開始を判断できるための指標の作成に向けて、2022年10月から2022年12月の期間は、高齢透析患者へ終末期医療とケアに関する意思確認を開始するタイミングに関する透析看護認定看護師の考えを明らかにするために、武庫川女子大学研究倫理委員会の承認（承認番号21‐40）を得て、インタビューガイドを用いた半構造化面接を透析看護認定看護師14名に実施し、得られたインタビュー内容の分析結果から質問項目を選定しアンケート調査票を作成した。アンケート内容は、問1)患者の身体状態に対してのタイミング(12項目)、問2)患者の生活の変化に対してのタイミング(7項目)、問3)患者の思い対してのタイミング(10項目)、問4)治療時期に対してのタイミング(16項目）、問5)高齢(65歳以上)透析患者へ意思確認する際の配慮や工夫点(7項目）、問6)意思確認に対する考え(5項目）の合計57項目である。アンケートによる調査は、4月17日に武庫川女子大学研究倫理委員会の承認（承認番号22‐102）を得て、透析看護認定看護師250名へ2回のアンケート調査（5段階評価のデルファイ法）を実施中である。また、本研究計画書作成に当たり、「終末期」の用語の定義のための文献検討を重ねた。本研究の結果だけでなく、透析看護認定看護師への調査結果の双方の視点から、高齢透析患者へACP開始を判断する際に活用できる指標を作成することは、ACPの推進と患者の望む人生を送るためのケア提供への一助となる意義があると考える。

The progress of the study title "ACP test for patients on dialysis at the terminal stage" and the preparation of the study proposal were carried out. On April 14, 2023, the application for research ethics review by the Research Ethics Committee of Mukukawa Women's University was submitted. Study Ethics Committee issued a reminder on May 27. The purpose of this study is to determine the start of ACP in HD patients by medical practitioners, and to determine the direction of index creation for HD patients during October 2022 and December 2022. The recognition of the Research Ethics Committee of Mukogawa Women's University (recognition number 21‐40) was obtained, and a semi-structured interview was conducted by 14 certified dialysis nurses using the INTAVIB website. The analysis results of the INTAVIB content were obtained, and the questioning items were selected. An investigation ticket was completed. Question 1) Patient's physical condition (12 items), Question 2) Patient's life (7 items), Question 3) Patient's thoughts (10 items), Question 4) Treatment period (12 items)(16 items), Question 5) High level (over 65 years old) dialysis patients 'consideration time points (7 items), Question 6) Meaning confirmation examination (5 items) Total 57 items. On April 17, 250 dialysis nurses were accredited by the Research Ethics Committee of Mukukawa Women's University (Accreditation No. 22 - 102), and the implementation of the five-stage evaluation method was under way. This paper reviews the literature on the definition of the term "terminal stage" in the preparation of this research proposal. The results of this study include the perspectives of the survey results of dialysis nurses, the use of indicators for determining ACP initiation in high-level dialysis patients, the promotion of ACP, and the significance of providing assistance to patients 'expectations.
