初年度である15年度は大きく2つのことを行った。1つは、文献レビューである。患者満足度調査と患者経験調査に関する、既存の理論研究、実証研究をレビューした。その結果、研究計画で検討していたとおり、患者経験調査の方が、医療機関評価を行う場合でも、医療機関の業務改善を行う場合でも、有効な方法であることを確認した。また、患者経験調査としては、米国CMS (Center for Medicare and Medicaid Services)が作成中のConsumer Assessment of Health Plans Survey (CAHPS)の調査票が参考になるとみられた。その調査票の項目には、例えば、「ナースコールを押したとき、看護師はすぐに来てくれましたか」や、「(入院中に痛みがあり、痛み止めの薬を欲しいと言った人について)痛み止めの薬を欲しいとあなたが言ったとき、病院の職員(医師、看護師など)はすぐに応じてくれましたか」などと、きわめて具体的で、患者の主観が入りにくい調査項目が多々あった。2つめは、調査項目(患者経験調査)の洗い出しである。医療機関に対するクレーム等を整理した既存文献のレビューを行うとともに、従来から患者クレームを整理している医療機関とその内容について検討を行った。さらに、特定の,医療機関で、実際に患者にインタビューを行い(パイロット調査)、その内容を整理した。実際に患者にインタビューをした経験では、具体的な質問を行えば行うほど、返答も具体的に返ってきており、質問紙も具体的であるほうが望ましいことが確認できた。
