妊婦の日常生活活動や社会参加への個別的な支援について、別の若手研究Bにおいて、2001年5月にWHOで承認されたICF (International Classification of functioning, Disability and Health:国際生活機能分類)を用い、妊婦の心身機能・身体構造の変化、活動や社会参加、環境因子の関連を検討した。本研究では、ICFの重要な視点である環境因子の影響を明らかにするため、社会的背景の異なる海外に在住する日本人の妊婦・褥婦について、生活上の問題と環境因子の関係をみた。日本企業から、多くの社員とその家族が中期の滞在をしている米国ミシガン州Ann Arbor市における、ミシガン大学での日本人向けの保健・医療システムにおいて、このサービスの開始前、開始時、現在の状況についてナースプラクティショナーへのインタビューから検討を行った。結婚、出産、子育て、子どもの教育など多くのライフイベントと課題を抱える家族を支える保健ニーズに対し、医師は、疾患の治療を重点とする医学モデル的アプローチをするのに対し、ナースは、その人の価値観や生活環境といった社会的要素を加味した社会モデルを中心としたアプローチをとるのが、特徴であった。妊婦が健康な生活を送る上での必要な環境条件として、質の高い医療の他、安心して受診できるというアクセシビィリティの確保が重要であった。海外では、言葉や文化の壁によって、これらが十分に満たされにくく、不安やうつなどの障害や疾患の早期発見・治療の阻害要因となっていることが示唆された。また、文化的な背景を考慮したサービスと地域での保健・医療サービスとの一体化の重要性も示唆され、国内の調査では得られなかったICFにおける環境因子の重要性が明らかとなった。また、これらの調査結果やデータベースを活用し、妊婦の多様化するライフスタイルに対応し、妊娠週数や合併症の有無などの個別的な状況に応じ、ニーズを把握し、利用できるサービスや制度の情報を含め、最適と思われるケアについての情報をウェブサイトで提供することとしている。
