本研究は、代表者らがこれまでに構築した多施設共同院外心停止レジストリ（基盤研究B、研究代表者 石見拓 H27～31）を継続・発展させ、健康関連QOL (Quality of Life)、患者・家族の治療選択に関わるデータベースを追加し、①患者・家族の意思を踏まえた院外心停止の治療戦略の構築、②QOL改善に寄与する治療、及び効果的治療対象の検討、③終末期の患者・家族の治療選択に関わる要因の検討を目的とする。令和4年度は、レジストリの運用を継続、データを前向きに登録・分析し、令和2年12月までの21,100例の症例についてデータを確定した。体外循環式心肺蘇生で治療された院外心停止患者の臨床的転帰、院外心停止患者の予後予測モデル、ST上昇のない院外心停止患者の心電図所見の診断精度などの研究について国際誌へ掲載、米国心臓協会年次学術集会などの国際学会でも報告した。令和5年度からパイロット研究を実施するQOLや終末期の患者・家族の治療選択に関する情報に関して、共同研究機関との定期的なミーティングで具体的な収集項目、収集方法を定めた。さらに、データ登録方法や登録情報の確認体制の改善のために、従来のWeb症例登録システムを見直し、新規のシステム会社(TXP medical㈱)との契約に至った。従来のシステムの入力画面は踏襲しつつ、以下の事項について改良を行った。①入力者がわかりやすいように入力項目の定義を入力画面上でも表示する、②入力ミスについて自動でアラートを出す仕組みを強化する、③新規収集項目は個別プロトコルとし既存のコア項目との違いをわかりやすくした。上記のようにデータベースを改良し、QOLや終末期の患者・家族の治療選択に関わるパイロット研究のための体制を整え、当初の計画通り実施する見込みが得られた。パイロット研究で抽出された課題について議論を進め、本登録を開始する予定である。

This study is based on the multi-facilities common hospital center established by the multi-facilities joint hospital and the representative of this study (Basic Research B, Taku Iwami H27～31), the health-related QOL (Quality of Life), treatment options for patients and families, additional treatment options for patients and families, construction of treatment strategies for out-of-hospital cardiac arrest , ② QOL improvement and treatment, and the effect of treatment and the target of the treatment, ③ End-stage patients and families, the treatment selection is based on the purpose of the treatment. A study on the diagnostic accuracy of electrocardiogram findings in patients was published in the Journal of International Journal of Cardiology and a report of the annual academic meeting of the American Heart Association was published in the International Society of Cardiology.
