高齢・障がいのために出所後の自立生活が困難な者に対する特別調整による支援を受けた出所者の刑事施設への再入所率は7.1％であったのに対し、特別調整辞退者では46.4％であった（平成30年度版犯罪白書）。特別調整が行われるためには「本人の希望」があることが条件となるが、適切な支援があることで、再犯防止や継続的地域生活定着を可能とすることが示唆されていることから、「本人の希望」の有無に関わらず、伴走型の重層的支援が望まれる。刑事施設出所直後からの支援に携わる保護観察官、保護司、更生保護施設職員を対象に、受刑経験者の継続的な健康生活支援に係るそれぞれの連携・協働について質問票調査を実施した。健康課題や障がいを持つ矯正施設入所経験者の対応について、対応経験がある健康課題や障がいの種類、連絡・連携・協働経験がある組織や職種の種類が最も多いのは地域生活定着支援センター職員であり、保護司は最も少ない。これは、職種に求められる役割として当然であり、保護司が担当する矯正施設入所経験者の犯罪傾向や背景要因は、保護観察官が対応する事例に比べて、複雑困難という訳ではないことも考慮する必要がある。連絡・連携・協働の方法として、「ケースカンファレンス」や「会議」の機会が最も多かったのは、地域生活定着支援センター職員、次いで保護観察官であったが、職種に求められる役割として当然であると言える。一方、定例会議等によって情報共有の機会やケースカンファレンスが実施される体制が構築されれば、自ずと連絡・連携・協働も活性化されることが期待される。すなわち、矯正施設入所経験者への支援において、保健分野との連絡・連携・協働が行われる場・機会の実体を構築することで、連絡・連携・協働が活性化され、当事者にとっても支援者にとってもメリットが増える可能性が示唆される。
