项目摘要

わが国では,先進諸国の中でもとくに多量の高度技術が急速に導入されているが,これらの医療技術の臨床的有効性と経済的効率についてほとんど検討が行われていない。そこで,わが国において,医療技術の普及および評価,それに対応する医療政策の特徴を,国際比較の中で明らかにすることを目的として研究を行った。具体的には,高度医療技術であるMRI(核磁気共鳴映像装置)および低侵襲性治療技術(内視鏡的治療ほか)を事例として用い,1)医療技術の総合的評価の体系化,2)日本における医療技術に対する評価と医療政策の特徴の解明,3)日本における医療技術の総合的評価の基準の設定について検討を行った。その結果,わが国では,こうした医療技術が,経済的効率だけでなく臨床的有効性も不明確なまま,臨床の場に普及していることが認められた。とくに,検査技術については,人口当りの普及率は世界最高であった。また,医療政策は,医療技術に対する総合的な評価と適正な利用に対する,科学的な政策決定を実施していなかった。その意味では,自由放任主義的な対応が,わが国の特徴であることが認められた。一方,欧米では,すでに公的な研究機関を中心として,各種の専門的な学会,民間財団,医療企業などさまざまな団体が,医療技術の総合的な評価を実施し,政策の決定にも影響を及ぼしていた。わが国でも、今後は,「評価なくして医療技術なし」の原則の下に,医療技術をテクノロジー・アセスメントの枠組みにより総合的に評価し,その結果を統合して具体的な医療政策を作り上げる社会的枠組みが必要とされていることが指摘された。
