项目摘要

The aim of this study is to examine the effect of prosthetic treatment on the salivary stress biomarker level of oral cancer patient. Sixteen patients were selected as subjects with partial maxillary bone defects. Prosthetic treatment with removable obturator was served to all subjects at Okayama University Hospital. The salivary Chromogranin A (CgA) were measured as salivary stress biomarkers during speech with prosthesis. To sixteen conventional denture wearers (for control), sampling of saliva was performed just before and after same speech intelligibility test. Speech intelligibility test score, visual analogue scale (VAS) satisfactory score and health scores were measured with and without prosthesis.The increase of salivary CgA level caused by speech test with prosthesis showed significantly higher value than the value without prosthesis (p<0.016). No significant differences were observed between those of the control group.The speech intelligibility test score and VAS satisfactory score showed the effectiveness of the prosthesis.From these results, it was suggested that the patients after oral cancer surgery still have mental stress even if the speech ability was recovered with prosthesis.
本研究的目的是检查口腔癌患者修复治疗对唾液应激生物标志物水平的影响。选择16例患者作为部分上颌骨缺损的受试者。在冈山大学医院,所有受试者都接受了可拆卸闭孔器的假体治疗。检测患者戴修复体发音过程中唾液嗜铬粒蛋白A(CgA)的变化。对16名传统义齿佩戴者(作为对照),在相同的语言清晰度测试之前和之后进行唾液采样。分别测量两组患者的语音清晰度测试评分、视觉模拟评分(VAS)满意度评分和健康状况评分,结果显示,两组患者的唾液CgA水平差异有统计学意义(P<0.016)。结果表明,口腔癌术后患者在使用人工修复后,即使恢复了语言能力,但仍存在心理压力。
