本研究では、ケロイド患者と非ケロイド患者の皮膚常在細菌叢の違い、またその違いがもたらす免疫系細胞の異常を解析することにより、新たな視点からケロイドの病態に迫っている。「ケロイド体質」の実態を新しいアプローチ法から解明し、ケロイドの予防法や治療法の開発につながる成果を目指している。[１．皮膚検体の集積と皮膚常在細菌叢の網羅的解析]ケロイド患者、非ケロイド患者から皮膚検体を回収しゲノムDNAの抽出、冷凍保存を行った。ある程度の検体数が集積された時点で、検体を外部解析機関に発送し、次世代アンプリコンシーケンス解析による皮膚常在菌の網羅的解析を行い、ケロイド患者に特徴的な皮膚常在菌層を同定した。[２．リンパ球のフローサイトメトリーによる解析]皮膚検体を採取した患者の血液検体を用いて、フローサイトメトリーによるリンパ球の解析を行った。表面抗原の解析により、Th1/Th2バランスや、新たなT細胞であるTh17やTh22に注目している。[３．単球・マクロファージ系細胞のサイトカインアレイによる解析]血液検体から単球・マクロファージ系細胞を回収して培養した。培養上清を回収し、単球・マクロファージ系細胞が分泌するサイトカインについて、サイトカインアレイによる網羅的解析を行った。ケロイド患者と非ケロイド患者で有意差のあるサイトカインについては、ELISAやReal-timePCRによるより詳細な分析を行った。前述のリンパ球の解析と合わせ、ケロイド患者における免疫系細胞の異常を同定し、皮膚常在細菌叢との関連についても明らかにした。[４．ケロイド未発症患者の前向きな追跡]ケロイド患者における皮膚常在細菌叢の特徴を明らかにし、ケロイド患者に特徴的な皮膚常在細菌叢を持つがまだケロイドは発症していない患者を前向きに追跡し、将来的にケロイドの発症が認められるかどうかを検証した。
