本研究は、厚生労働省が通知している育児に向けて出産前から支援体制の強化を必要とされる特定妊婦に、生活行動に含まれる感情や思考をインタビューし、生活行動を規定する胎児への愛着について明らかにすることを目的とした。研究準備期間は、周産期メンタルヘルスや特定妊婦が抱える課題、胎児への愛着、妊婦の生活行動に関する文献を検討し、関連学会や研修への参加を通して近年の動向や最新の知見を把握した。特定妊婦は心理社会的ハイリスクにおける8つの視点から分類されるが、本研究は、①支援者がいない妊婦（未婚または親族など身近な支援者がいない妊婦）、②望まない妊娠をした妊婦（育てられないもしくはその思い込みがある、婚外妊娠の妊婦、経済的困窮がある妊婦）を対象とした。北海道内の8か所の産科施設に研究依頼したが、対象者の選定および同意に困難を来す等の理由で実施まで至らず、最終的に予定した研究機関を3か月延長し、2か所の産科施設より同意が得られ、1名にインタビューを実施した。インタビューデータを内容分析した結果、特定妊婦に分類される心理社会的ハイリスク妊婦であっても、妊婦を取り巻く家族支援サポートの充実によって胎児の順調な発育に配慮した健康的な生活を維持でき、産後の育児を見据えて生活を整える行動がみられた。このことから特定妊婦に分類されるいずれもが妊娠期から支援強化の必要なハイリスク妊婦ではなく、妊婦を取り巻く家族を中心とした支援体制の査定およびその強化が特に重要であることが明らかとなった。今後は心理社会的ハイリスク妊婦を支える周囲サポートの実態と、サポートの有無による影響、さらにはサポートを強化する支援体制の構築に焦点を当てた研究が必要があると考える。
