エキシマレーザーを用いた近視矯正手術や、角膜混濁などの病変部の切除を目的とする治療的角膜切除は日本にもそのレーザー発振装置と技術が導入されている。エキシマレーザーによる角膜の切除後にはほぼ全例において、角膜上皮混濁が出現する。上皮下混濁はコントラスト感度の低下や、グレア-、ハロ-などの視機能の低下を生じる。比較的強度の上皮下混濁はそれに加え屈折度数の退行(regression)や矯正視力の低下を伴うことがある。我々はエキシマレーザー近視矯正手術後に低矯正となった症例における再照射の安全性と有効性を調べた。対象症例は術後12ヶ月以上経過し、かつステロイド点眼による屈折の回復を認めなかった10例10眼とした。再照射を必要とした原因の内訳は低矯正2眼、低矯正と上皮下混濁の合併4眼であった。最初のPRKから再照射までの期間は14〜21ヶ月(平均16.5ヶ月)であった。一度目PRKの術前平均屈折度数は-13.58±5.19D、平均矯正度数は9.10±1.39であった。再照射前の平均屈折度数は-6.88±2.92D、平均矯正度数は4.83±2.07Dであった。平均観察期間:8.66±5.66ヶ月において、裸眼視力は全例で上昇し、2段階以上眼鏡矯正視力の低下は認めず、全例で不変であった。混濁の程度は術前平均1.07±1.24が術後平均0.83±0.25と改善した。6ヶ月以上観察可能であった6眼の内、目標屈折値に対し±1D以内が5眼(83.3%)であった。術前後における上皮バリアー機能、上皮と内皮細胞の平均面積に有意な差は認めなかった。術前のSRIと比較して、再手術後3ヶ月の時点では有意差を認めたが、術後6ヶ月以降は術前と比較して有意差を認めなかった。現時点において、エキシマレーザー近視矯正手術における再照射は有効かつ、安全と考えられた。
