患者報告型アウトカムとは、痛みなどの身体症状をはじめとした精神症状、社会的な問題などを患者から直接聞き取ることである。高齢者からの患者報告型アウトカムの取得は身体機能や認知機能の低下により、聞き取り形式を取らざるを得ないことが多く、医療者の負担などが臨床現場における実装のハードルになっている。本研究ではタブレット型端末とIT技術を駆使した新しい患者報告型アウトカムの取得方法を開発する。IT技術としては画像認識技術、音声認識技術、自然言語処理、人工知能の組み合わせを想定しており、IT企業および東北大学病院と共同で開発する。１年目にタブレット型端末の開発し、２～３年目に高齢患者を対象とした実装試験を行い評価および修正、電子カルテとの連携に挑戦する。開発したタブレット端末は原理的には高齢患者以外のすべての患者や市民を対象に利用するものであり、患者報告型アウトカムに限らず、日常の看護ケアに活用できるものである。2022年度は主として音声認識技術について検討した。患者報告型アウトカムとしてはIPOS（Integrated Palliative Care Outcome Scale）を用いて、健常人を対象としたパイロット調査後に診療所や高齢者施設でリクルートした高齢患者100人の症状聞き取りの録音データを収集した。2023年度は録音データの下処理を行ったのちに、自然言語処理技術および人工知能の技術を組み合わせて、その判読性能を分析する予定である。
