项目摘要

初年度にオミックス解析のうち脂質解析による研究成果を論文発表した。高度肥満症患者の術前・術後6か月時の脂質解析による遊離脂肪酸（FFA）の変化と体重減少および代謝改善指標との間に相関があることを証明した。特に、リン脂質は腹腔鏡下スリーブ状胃切除術（LSG）後に増加する傾向があり、アラキドン酸を含むPC(18:1e_20:4)は非アルコール性脂肪性肝炎（NASH）合併群においてのみ有意に増加した。高度肥満症に対するLSGは、アラキドン酸カスケードの進行を抑制する可能性があり、肝細胞内での持続的な炎症を消退させる可能性がある。また、PC(18:1e_20:4)はNASH群でのみ術後に有意な増加を認めたことから、NASHの診断や治療効果判定のためのサロゲートマーカーとして使用できる可能性があると結論付けた（Takahashi N, Sasaki A, Umemura A, et al. Biomedicines. 2023）。具体的な結果は、以下に記載する通りである。NASH群において、術後6か月で肝組織の脂肪化割合、NASH activity score （NAS）の脂肪化スコア、NASの炎症スコアは有意に改善した。脂質解析により全患者で共通して検出された189種のFFAのうち、肝組織から検出された53種のリン脂質と相同性が見られ、術後に3種のリン脂質の有意増加が見られた。PC(18:1e_20:4)はオレイン酸とアラキドン酸が結合した脂肪酸で、NASHの陽性診断とのROC曲線において，AUCは0.707，カットオフ値は20711.3 μg/mL，NASHの陽性診断率は81.6%と高値であることを示した。
