柴田は、メサンジウムを主座とする進行性糸球体障害のモテルとして、基底膜由来の糖タンパクあるいは糖ペプチドによるラット腎炎を提唱した。このタイプの腎炎はメサンジウム内に、myeloid bodyを形成するという特徴をもつ。一方メサンジウム内のmyeloid body形成は、リソソーム酵素ceramide trihexoside α-galactosidase欠損によるFabry病腎炎や、アミノグリコシド抗生物質投与による不可逆的な腎障害の際にしばしば認められる。このタイプの腎炎の発症機構を解明することを最終目的として、リソソーム系の発達しているヒト皮膚繊維芽細胞に、腎炎惹起性糖タンパクあるいはアミノグリコシド抗生物質(ゲンタマイシン)を投与し、そのリソソーム酵素への影響を調べると共に、myeloid bodyの形成を電子顕微鏡で観察した。またメサンジウム細胞を培養し、これを用いてmyeloid body形成を観察した。その結果腎炎惹起性糖タンパクあるいはゲンタマイシンを投与した。ヒト皮膚繊維芽細胞と、ラット系球体より培養したメサンジウム細胞内に、投与後1日から3日で多数のmyeloid bodyが観察された。この時リソソーム酵素のうち6種のグリコシダーゼ活性には大きな変動は認められなかった。しかし、りん脂質代謝系のリソソーム酵素である酸性スフィンゴミエリナーゼの著しい低下と、酸性ホスホリパーゼ活性上昇が認められた。また、酸性エステラーゼおよびリパーゼのの低下が認められた。りん脂質の分析により、スフィンゴミエリンの蓄積は認められなかったが、ビスホスファチジン酸の蓄積とエステル型りん脂質の量的変動が認められた。従って、腎炎惹起性糖タンパクやゲンタマイシンを投与すると、リソソームの脂質代謝系に変動が起る事が判明した。この変化は、繊維芽細胞にとっては、致死的ではないが、メサンジウム細胞にとっては不可逆的な障害であり、進行性の系球体障害の原因となりうると推察された。
