Our method was designed to utilize liver graft obtained from a living and related donor in canine partial liver transplantation.The results obtained are summarized as follows:1) The volume of graft necessary for survival was investigated by in vivo perfusion method. left lateral, left central lobe and processus of caudate lobe were perfused safely and 6 out of 13 cases survived for more than 3 months. From this result, these 3 lobes from sacrificed donor were orthotopically transplanted in total hepatectomised dogs, and 5 out of 8 cased treated by ciclosporin and methylpredonisolone survived for more than 7 days and the longest survival time was 40 days.2). In order to use a living donor, left central and left lateral lobe were perfused in vivo and transplanted orthotopically. Eight out of 15 donors survived for more than 3 weeks and the operation for donor was proved to be done safely.Six out of 15 recipients survived for more than 7 days and the longest survival time was 10 days at the present time.Thus, the mothod of in vivo partial liver perfusion was thought to be applicable clinically, and it was further suggested that the recipient could survive with only left lateral and left central lobe, when the weight of donor and recipient were nearly equal.
本实验采用活体供肝和亲属供肝进行犬部分肝移植,结果如下:1)采用活体灌注法研究供肝存活所需的体积。左外叶、左中央叶及尾状叶突灌注安全,13例中6例存活3个月以上。由此可见,将这3个供肝原位移植于全肝切除犬,环孢素和甲基强的松龙联合治疗的8例中有5例存活7天以上,最长存活40天。为了使用活体供体,在体内灌注左中央叶和左外侧叶并原位移植。15例供者中8例存活3周以上,供者手术安全,15例受体中6例存活7天以上,目前存活时间最长者达10天,认为在体部分肝灌注法具有临床应用价值。当供、受体体重相近时,受体可仅保留左外叶和左中央叶存活。
