開頭手術患者7名に、脳圧下降の目的でマンニトールを使用し、血漿浸透圧Na,K、人心房性Na利尿ホルモン,抗利尿ホルモンの測定と尿量,尿中Na,Kの排泄を観察した。その結果、マンニトール投与により血漿浸透圧の上昇と血漿Na,K濃度は下降した。マンニトール投与後30分の尿量は約3倍となり、Na排泄量は約4倍に増加し、K排泄量も増加傾向にあった。人心房性利尿ホルモンは、マンニトール投与により下降した。抗利尿ホルモンは、マンニトール投与後、変化が見られなかったが2名だけで上昇が見られた。これらの結果をまとめると以下の様である。マンニトール投与による細胞外液量とくに循環血液量の増加や血漿浸透圧の上昇により、心房性Na利尿ホルモンが増加して利尿作用の一因となっているのではないかと予想したが、結果は逆であり、マンニトール投与時の利尿には心房性Na利尿ホルモンは関与していないと考えられる。心房性利尿ホルモンの下降の原因は、希釈や分泌低下などを検討する必要がある。マンニトール投与による血漿浸透圧の上昇は、抗利尿ホルモンの分泌刺激になると考えられており、抗利尿ホルモンの増加が予想されたが、2名を除き変化が見られなかった。このことから、抗利尿ホルモンの分泌刺激は、血漿浸透圧の増加の内容により異なると考えられ、Naが上昇する必要があると考えられた。抗利尿ホルモンの濃度についても、同様に希釈の影響を補正して、分泌あるいは排泄状況を更に調べる必要がある。
