(目的)我々はIUGRの治療に対し, 静注用脂肪乳剤の有用性を報告してきたが, 今回脂肪乳剤投与後の母児間の脂質, 糖, 及びアミノ酸代謝さらに, 胎児成長因子であるIGF-1とインスリンの動態について検討した.(方法)対象は正常妊婦で予定帝切施行3時間前に脂肪乳剤400mlを点滴静注した. 分娩時母体血, 臍帯血, 羊水を摂取し, TG, FFA, 糖, 遊離アミノ酸インスリン, IGF-1を測定した.(成績)本剤投与後, 臍帯血中FFA, TGのリノール酸分画は非投与群に比し, 増加したが, FFA, TGの濃度には有意差を認めなかった. 母体血中アミノ酸濃度は非投与群217.2μmol/ml, 投与群238.7μmol/ml, と上昇傾向を認めたが, 有意差は認めなかった. 臍帯血中では非投与群304.9μmol/ml, 投与群231.3μmol/mlと有意の低下(P<0.1)を示し時にアラニン, バリンなど糖原性アミノ酸濃度が低下した. 一方, 母体血中グルコース値は非投与群で96.2mg/dl, 投与群で140.8mg/dlと有意に増加した. (P<0.1)インスリン値は平均値で減少したが有意の差ではなかった. 臍帯血中グルコース値には有意の変化はなくインスリン値が非投与群6.7w/ml, 投与群83w/mlと有意に上昇した. (P<0.1)IGF-1は母体血中, 臍帯血中ともに投与群で有意の差はなかった.(結語)今回の本剤投与方法により(1)胎児側にリノール酸をより多く供給しながらも高脂血症を惹起しない. (2)又血中遊離アミノ酸, 主として糖原性アミノ酸を減少させる. (3)インスリン分泌を軽度刺激し胎児側での血糖に変化をひきおこさず. (4)IGF-7の分泌に影響を及ぼさない事がわかった. 脂肪乳剤投与は胎児側でのエネルギー代謝を脂質代謝へ傾斜させ, 蛋白からのアミノ酸動員及び, 糖新生を軽減させているものと推察された.
